I, hereby, declare that, the entrance made by me with my consent in the Application Form submitted to the Institute are complete, authentic and true and based on records and accept to present the original documents immediately upon demand by the concerned authorities of the Institute.
I, hereby, promise to abide by the sanctioned rules and regulations, concerning all the discipline, attendance,management etc. of the Institute, and also to follow the Code of Conduct prescribed for the Students of the Institute, as in force from time to time and following changes/modifications/amendment made to that.
I also acknowledge that, the Institute has the authority for taking correctional actions against me for violation and/or non-compliance of the same.
I understand that, 80% attendance in classes is obligatory and I commit myself to adhere to the same. I also understand, in case my attendance falls short, for any reason, the Institute may take such action as may be deemed fit and proper. Also I have to maintain the class time and schedule as per the Institute's rule.
I, hereby declare that, I will neither join in any coercive agitation/strike for the purpose of forcing the authorities of the Institute to solve any problem, nor I will participate in any activity within or outside the Institute which has a tendency to disorganize the image, goodwill, peace and tranquility of life of the Institute campus and/or its establishments.
I, hereby declare that, the copyright of the content, recorded audio, video and any material created by me during my occupancy at the institute will be the property of the institute and that I will have no claims on such material.
I, hereby declare that, both I will not indulge in, nor tolerate ragging, in any form, even in words or purposes, I will be solely responsible for my involvement in any kind of undesirable / in disciplinary activities outside the campus, and shall be liable for reasonable punishment as per the law of the land. I further understand that, the Institute shall in no way provide any support to me and will not be held responsible for any of my such action.
I, also declare that, I am not suffering from any serious/contagious sickness and/or any psychiatric / psychological disorder and I have consulted an authorized doctor/hospital and have their approval to participate in all physical, mental and emotional activities conducted during the course of the institute.
I, further declare that, my admission may be cancelled, at any stage, if I am found incompetent and/or the information provided by me are found to be incorrect.
I, hereby take forward to apprise the Institute via an email to support@fiftbd.com about any changes in information capitulated by me, in the Application Form and any other documents, including change in addresses and phone numbers, from time to time. As the assortment is limited to a fixed number of aspirant, no requests for refund of fees for the programme will be accepted.
I, Hereby,pledge to not consume alcohol or any other drugs before the class or during the class hours. If found culpable, my admission can be terminated.
